Legal Name vs Familiar Name was created by
I am wanting to fill out the application but I don't want to misrepresent myself.
I was assigned male at birth, but I identify as (gender) queer femme and use she/her/hers pronouns. All my legal documents recognize me as legally male and use a name that I do not go by. As of this moment, I am not changing my name and don't when or if I will legally change my name (I am also aware that if I do, I will need to notify an Intake Officer).
My question is, other than where it asks me for my legal name
Can I use my familiar name everywhere else? Am I allowed to use the "Ms." prefix?

How should your name read on any certificates or ID's? (Legal Name)

Otherwise than where your legal, given name as it appears on your birth certificate is required (for legal purposes), you may ask to be addressed as you like. If you should like to go by Ms., that is fine too - it is just that we generally do not call one another like that.

I'm just asking in terms of filling out the application correctly because it ask me my legal name, then what prefix I use, and then asks me for my full name but doesn't state whether it has to be my legal name or if it can be my familiar name.
