Confidant Session Form

Follow up question as it's been a few days:

Has the link been tested, does the contact form work, and if it has been used, does it fully meet the needs of those contacting us? If not, what sections should be expanded to better meet the needs of those who use it?

It has been tested and works but hasn't been used yet.

I'd just like to add that this is awesome to have in the Navigation bar. Thanks to all for the hard work and getting it done.
